Presentation Abstract:

This presentation shares research that showed observation of in-class healthcare simulation and being in the primary nurse role improved clinical judgment skills.

Background: Literature reveals that high-fidelity simulation-based experiences in the simulation lab setting have a significant effect on clinical judgment development in pre-licensure nursing students. Yet, the efficacy of using in-class simulations to develop clinical judgment is understudied. 

Aim: To address these gaps in the literature, a quasi-experimental repeated measures study was conducted. The aims were to determine if interprofessional simulations shown in the classroom improve clinical judgment skills in the simulation setting and if serving in the primary nurse role improved demonstrated clinical judgment abilities among pre-licensure students.

Theoretical Framework: This study was guided by Tanner’s Integrative Model of Clinical Judgment (2006) because the effect that interprofessional simulations in the classroom have on how nursing students notice, interpret, respond, and reflect in clinical simulations was evaluated. Method: Three clinical simulations were observed in the classroom over a semester and clinical judgment was measured three times with the Lasater Clinical Judgment Rubric in a simulated setting among 45 study participants. 

Results: Clinical judgment skills increased (p < .001) after the in-class simulations. Significant differences were seen between the scores for primary nurse role (M = 29.71, SD = 3.770) and supporting nurse role at the third measurement point (M = 24.57, SD = 5.071); t (42) = 3.792, p < .001. 

Conclusion: Observation of in-class simulation and being in the primary nurse role improve clinical judgment skills.

Laura Klenke-Borgmann, PhD, RN, CHSE

Clinical Assistant Professor, The University of Kansas School of Nursing
Dr. Klenke-Borgmann received a PhD from Villanova University in 2020. She is a Clinical Assistant Professor at the University of Kansas School of Nursing in Kansas City, KS, where she currently teaches in simulation and the classroom. In August 2020, she was the recipient of the University of Kansas Health System Patient Safety Through Simulation fellowship. She is an independent consultant for Shadow Health. Dr. Klenke-Borgmann has published in several peer-reviewed journals, including Clinical Simulation in Nursing in the February 2021 issue. She is passionate about advancing experiential learning strategies for nurses and nursing students that develop clinical judgment and prepare them for clinical practice through research on the effect of observation of in-class simulation and interprofessional simulation.
